Affiliate/Meeting-in-Conjunction Room Request Form

IMPORTANT DEADLINES

  • Request Forms received on or before January 2, 2015 will be listed in the Formal Printed Program
  • Request Forms received on or between January 3, 2015 and January 30, 2015 will be listed in the Addendum
  • Request Forms received after January 30, 2015 will not be listed in AAS print publications

The AAS can arrange for the equipment listed above to be set in each room but this equipment is NOT complimentary. The full A/V package of one (1) LCD Projector and Screen & one (1) Microphone is $325.

PLEASE NOTE: If you have requested A/V equipment, you will be responsible for the cost of equipment rental and labor charges; please make sure you give accurate information so that we can assign your group a space where you can share the cost with other groups. AAS makes an effort to decrease the A/V cost to our affiliate groups by offering the option to share the costs with other groups. Additional equipment needed beyond the basic package as listed above must be ordered directly from the hotel and your group will be billed separately by the hotel.

Saturday, March 28 1:00pm – 2:30pm (Due to short turnaround on either before and after this time slot, large lunch catering orders are not allowed in this time slot)
